Not more than 2 employees, includes real estate or insurance: $130.00

PROFESSIONAL – CLASS IV

Includes doctors, attorneys, dentists, accountants, veterinarians, pharmacists, morticians: $225.00; $100 for each additional professional

RETAIL – MANUFACTURING – SERVICE – CLASS V
With 2 but not more than 4 employees: $150.00
With 5 to 8 employees: $200.00
With 9 to 15 employees: $250.00
With 16 to 25 employees: $300.00
With 26 to 75 employees: $350.00
With 76 or more employees: $400.00

NURSING HOME – CLASS VI
No employee number charge: $300.00

RESTAURANTS – CLASS VII
No employee number charge: $150.00
